Adding a basic feedback form to pages in a Notion site

I have been slowly building out a public facing Notion site with documentation of our tools, and I'd like to add a single question feedback form to most pages on that site.

It doesn't need to be fancy at all -- embedding a Baserow form would work fine -- but I'd like to capture the page name so I can see which section folks are responding to.

Has anyone done this? Is there a streamlined way to capture the page name or URL?

You can try services like Tally and Youform, with both of them and also with similars ones you can do what you are looking for.
