How do I create a simple form to capture email addresses

I just started using notion. I want to create a simple way to capture email addresses. Suggestions

You will need an external service for having forms inside Notion.

I believe that Tally can do the job you are looking for, but there are more out there and if you do a simple web search, you can find other options.
